"The unprejudiced observer- well aware of the futility of transcendental speculations which can receive no confirmation from experience -be his powers of penetration ever so great, takes note of nothing in every individual disease, except the changes in the health of the body and of the mind (morbid phenomena, accidents, symptoms) which can be perceived externally by means of the senses; that is to say, he notices only the deviations from the former healthy state of the now diseases individual, which are felt by the patient himself, remarked by those around him and observed by the physician. All these perceptible signs represent the disease in its whole extent, that is, together they form the true and only conceivable portrait of the diseases."

Note: [I know not, therefore, how it was possible for physicians at the sick-bed to allow themselves to suppose that, without most carefully attending to the symptoms and being guided by them in the treatment, they ought to seek and could discover, only in the hidden and unknown interior, what there was to be cured in the disease, arrogantly and ludicrously pretending that they could without paying much attention to the symptoms, discover the alteration that had occurred in the invisible interior, and set it to right with (unknown) medicines, and that such a procedure as this could alone be called radical and rational treatment.

Unprejudiced Observer: Dr. Hahnemann used this word in especial sense, this term indicates a quality of physician, the unprejudiced observer is who, do not use his or her previous experience resolving a new case. every case is a new page of the book and need to read carefully. this term unprejudiced observer advocates the principal of individuality. A Homoeopathic physician must be an unprejudiced observer. 

futility of transcendental speculations: Unusefulness of empty theories. Aphorism 6 is connected with Aphorism 1, where Dr. Hahnemann discussed about the theoretic medicine (Useless concept). here Dr. Hahnemann addressed to the physician that "if you are true follower of Organon of medicine, you well know, how the transcendental speculations are useless". it is not the job of Physician to form them.  

Story form (Explanation) 

In this Aphorism, Dr. Hahnemann offers various instructions to his followers. This Aphorism enlightens and reveals many important facts. Foremost among them is the principle that a homoeopathic physician must be an investigator, approaching every case as a new case. It is not the physician’s duty to rely on previous experience when solving a new case. The physician must possess the power of perception, so that he can perceive and record every deviation from health—whether arising from body, mind, or spirit, externally or internally. Apart from these perceptible signs and symptoms, which can be perceived by the physician, observed by attendants, and felt by the patient himself (morbid phenomena), nothing else needs to be investigated, such as blood reports, stool and urine examinations, CT scans, or MRI. If the physician has sufficient power of perception and the ability to analyze data, then these external signs and symptoms represent the true portrait of the disease in its whole extent.

Understanding of the Organon of medicine is now easy with explanatory note, thanks HomeoEra for such an easy clarification. this aphorism gives me a great teaching that only perceptible sign and symptoms just sufficient to grasp the full idea of disease in its whole extent.
